Lady Guinere is the Captain of the Guard in Yew in Ultima V: Lazarus.

Appearing sweet and naive, her bubbly demeanor hid a cold and calculating soul. She told the Avatar that her job as Captain of the Guard made her third in command to Blackthorn, and this was a difficult job because she intended on seeing to it that the people of Britannia no longer paid lip service to the Virtues, but that they would be rigidly forced upon the populace. She claimed that she believed this to be in the best interest for Britannia, and that she took her job most seriously. She briefly mentioned the prisoners in Yew, and the jailer Gord, whom she suggested was kind and took good care of the prisoners, although Gord briefly mentioned the possibility of a sexual relationship between the two characters.

The Avatar could learn through Widow Hahna and Lord Tenneth that Lady Guinere had once been a citizen of Minoc, where she was discovered by Fenelon at the Poor House who put her to work as a seamstress. Lady Guinere had then entered into a sexual relationship with the married Lord Tenneth with the intent of blackmailing him to keep the relationship a secret. She further tried to blackmail Widow Hahna regarding the circumstances of her husband's death. The two with the assistance of Fenelon had attempted to have the Captain killed by Lord Dalgrin, but Lady Guinere survived and became Blackthorn’s guard captain, returning to Minoc to punish those who had previously sought her death.

If the Avatar should agree to assist Judge Dryden, Lady Guinere would tell the hero that a band of orcs were threatening Skara Brae and Yew, and that she required a shipment of arms from Chamfort the blacksmith to defend from this threat, but the smith had refused to be of assistance. If the Avatar convinced Chamfort that this threat was genuine, he would provide the arms and Lady Guinere proved most grateful.

For her second quest, she asked the Avatar to provide an Amulet of Turning, saying that Lord Simon and Lady Tessa had known of the magic required to make this artifact, but the two had turned traitor. Should the Avatar produce and Amulet of Turning, Lady Guinere would show her gratitude by offering to kiss the Avatar.

The Lady's third quest required the Avatar to seek out the leaders of the Resistance, and infiltrate that organization in order to bring its leaders to Blackthorn’s justice. The hero could choose to betray Landon to Lady Guinere, causing his death.
