More fields for complicated references

I was having trouble extracting U7.5 references from its usecode. I think the "keywords" field is weak for this purpose and the template might be strengthened by some more optional fields that make it possible to clarify references in text that can't easily be located through keywords. In general, I think comment fields would help all references, because it's difficult to know exactly what is being justified with the information currently given, but for some reason, that's not how formal references are made.
