The Inquisitor is the torturer for the Oppression in Yew in Ultima V: Lazarus.
This strange, pale, hairless woman was employed by Judge Dryden with the Oppression to extract a confession from all of their prisoners. She was willing to share very few words with the Avatar, only mentioning that she looked forward to "working" with the hero in the future.[1]
Blackthorn, in a moment of remorse while alone in his chambers, will reveal that the Inquisitor used to be his good friend Shaana. After her death in New Magincia, some force of evil magic was used to bring her back as this soulless character.[2]

In The Ultima 6 Project, a tombstone in the cemetary of Empath Abbey reads, "In death defiled, her evil fetch did torment many a hapless wretch. The spell's undone and with release, brave Shaana's soul may rest in peace." This means after the demise of the Shadowlords, her body, no longer sustained by the dark magic, returned to being a corpse and she finally found her peace.
